چکیده

Background 12-lead Electrocardiogram (ECG) is a clinical standard for patient physiological monitoring. An MRI-conditional 12-lead ECG should permit detection of acute myocardial ischemia during MR imaging or MRI-guided therapy, which may improve the handling of patients with ischemic histories. MRI visualization of ischemic episodes can also enhance the understanding of ischemic progression. Previously an MR-conditional 12-lead ECG system was presented. The system was equipped with GradientRamp&RF (GR&RF) noise removal hardware & Magnetohydrodynamic (VMHD) voltage-removal software that improved ST segment visualization [1]. The study objectives were to (1) validate simultaneous 12-lead ECG monitoring & cardiac MR imaging in human subjects; (2) detect S-wave to T-wave (ST) ECG elevation & perform MR imaging of a Left Anterior Descending (LAD) balloon occlusion from the onset of ischemia to death in a swine model.

Cardiac MRI with concurrent physiological monitoring using MRI-compatible 12-lead ECG

Background High fidelity 12-lead ECG is important for physiological monitoring during cardiovascular interventions. A dominant ECG R-wave is essential for synchronizing cardiac MRI. Obtaining the real ECG in MRI is challenging due to a superimposed Magneto-Hydro-Dynamic (MHD) voltage (VMHD) [1], & strong induced voltages from MRI switched gradients. Detecting acute ischemia by ST segment [2] is...

3T cardiac imaging with on-line 12-lead ECG monitoring

Background We previously demonstrated rapid detection of acute ischemia inside MRI using a prototype MRI-conditional 12-lead ECG system [1] equipped with hardware to remove artifacts produced by the MRI gradients. We also synchronized [2] high-field (1.5-7T) scanners using triggers from 12-lead traces following Magneto-HydroDynamic voltage removal. A commercial 12-lead ECG system might allow pe...

Morphology-matching-based R-wave detection for noise-robust ECG gating

Background Accurate ECG R-wave detection is crucial for cardiac gating in MRI. However, in high-field MRI systems, it is hard to detect R-waves in ECG signals accurately, because the amplitude of the ECG signal may be smaller than that of the noise induced by the MRI system. To overcome this issue, existing studies have focused on (a) acquiring additional ECG signals or on (b) improving the R-w...

A Novel Point-of-Care Smartphone Based System for Monitoring the Cardiac and Respiratory Systems

Cardio-respiratory monitoring is one of the most demanding areas in the rapidly growing, mobile-device, based health care delivery. We developed a 12-lead smartphone-based electrocardiogram (ECG) acquisition and monitoring system (called "cvrPhone"), and an application to assess underlying ischemia, and estimate the respiration rate (RR) and tidal volume (TV) from analysis of electrocardiograph...

Summation and Cancellation Effects on QRS and ST-Segment Changes Induced by Simultaneous Regional Myocardial Ischemia

Simultaneous ischemia in two myocardial regions is a potentially lethal clinical condition often unrecognized whose corresponding electrocardiographic (ECG) patterns have not yet been characterized. Thus, this study aimed to determine the QRS complex and ST-segment changes induced by concurrent ischemia in different myocardial regions elicited by combined double occlusion of the three main coro...
